Inmate Medical Records

Medical records can only be released to the individual that was incarcerated. We cannot release records to anyone unless there is an authorization for release of medical records. (Required by the Health Insurance Portability and Accountability Act of 1996 – “HIPPA” – Public Law 104-191)

In order to obtain a copy of medical records on file, the individual that was incarcerated must bring a current and valid government issued identification to the Jail Administration Office, at the Clayton County Jail, and fill out a medical records request form.

Tuberculosis (TB) and/or Human Immunodeficiency Virus (HIV) test results are only available for individuals incarcerated for more than 72 hours. If the individual was released prior to 72 hours the test results may not be on record.

If you are an attorney or representing a medical facility you may fax a typed request on letterhead to the Medical Records Section. The fax number is listed in the contact information below.

There is a fee to receive medical records. The fee schedule is determined by the Medical Section and is as follows:

– Administrative Costs: $25.88

– Certification Fee: $9.70

– Copying costs per page (1 – 20 pages): $0.97

– Copying costs per page (21 – 100 pages): $0.83

– Copying costs per page (100+ pages): $0.66

Once the medical records request has been processed by the Medical Section (usually 3 – 5 business days) the amount of the fees will be calculated. The individual requesting the records will then be contacted and informed the records are ready. The Individual will need to bring a money order for the amount of the fees to the Jail Administration Office in order for the records to be released. Cash will not be accepted.

The Jail Administration Office receives requests for the Medical Section for security purposes. The Medical Section will not take telephone calls from outside the facility. The Jail Administration Office is open Monday – Friday, between 8:00 a.m. – 4:00 p.m.
